1. Fill in the CDSBC application form a MINIMUM of 4 weeks before your first surgery. Note that the application form requires a specifically sized photograph and needs to be notarized. 2. Confirm that you have the minimum $2 million malpractice that covers you in BC/Canada. If you DON'T, you need to fill in and submit the CDSPI form as well. 3. Minimum 3 weeks before surgery, contact the CDSBC [Karen Walker] to request a temporary license for the specific date of your surgery.
